Self Screening Questionnaire

Below is a Medical History Questionnaire that can be used as a precaution to screen a student athlete for possible underlying heart disease and to prevent problems in the future. The first section asks questions about personal history and the second asks about Family History. If most of the answers are yes, we recommend you go to the doctor  and ask them if they think you should get a more thorough genetic test.
